Schedule Pickup
Dear Frankford
Parents and Guardians:
We are so excited
to see you and your students for the 2011-2012 school year! Please plan to join
us for Schedule Pick-Up on August 11 and 12. This event is a Back-To-School
must: buy your school supplies, purchase Spirit and Athletic Wear, confirm bus
routes and meal payment plans, turn in all required forms, sign-up to
volunteer, and, most importantly, your child will be able to pick up that long
awaited schedule and locker assignment.
In addition to the following forms, you
MUST complete the PISD Student Emergency Form ONLINE between August 1 and August
9 for your students to receive their schedules. Please bring proof that
you have filled out the form.

Go over your
checklist for this event and bring it to Schedule Pick-Up with your completed
forms. Remembering to bring these papers with you and completing your PISD
Student Emergency Form online will really speed up the process for you and your
student. We will be accepting Visa, Mastercard, cash, or checks made payable to
FMS PTA, so remember to bring your preferred method of payment. Best of all,
Schedule Pick-Up is a tax-free event!
Volunteer Criminal Background Check Renewals
PISD requires that all volunteers have a completed Criminal History
Check and PISD Volunteer Information Form in order
to participate in any volunteer opportunity at PISD schools. Please log on to the PISD
website and complete this process. Applications for the
upcoming school year may be completed starting July 1.
Log on to www.pisd.edu, then click on "Volunteer Application"
on the right side of
the page. Follow the prompts to complete. A new application must be completed each school year.
Volunteers who completed this form for the previous school year created a
User ID and Password. If you do not remember these, simply click on the appropriate
spot on the first screen of the application to get this information.

3. Utility (or similar) bill showing proof of residency at the address that they register with.
